About the Role

Ottawa has a growing immigrant population who are at risk of developing diabetes and other chronic diseases, but they encounter many barriers when trying to access health care. Also, other populations are at higher risk of developing diabetes and other chronic diseases. The SCREEN program focuses on building a screening, referral and education model integrated across the Champlain region CHCs that provides integrated chronic disease risk assessment services for high risk and immigrant populations.

The community health worker will work collaboratively with community health and resource centres, other community based organizations, chronic disease partners, and community leaders and volunteers to identify needs and capacities, develop strategies and implement chronic disease screening event models as per the communities’ unique requirements. The program currently offers diabetes screening with plans to expand to include other chronic diseases. Activities of the Community Health worker include community organizing, planning and leading community-screening session, building community networks, and communicating within and across communities.

The community health worker is key point of contact to reach the community and community leaders and mobilize volunteers to organize screening sessions, referral, integration, health promotion and advocacy.

Requirements

Essential

  • Degree in health or social sciences or equivalent combination of education and experience.

  • Demonstrated experience in community engagement.

  • Ability to work well with marginalized groups.

  • Excellent organizational and interpersonal skills

  • Knowledge of and experience with community groups made up of high risk and immigrant populations and the organizations that serve them

  • Demonstrated skills in planning, organizing and facilitating community events

  • Knowledge and experience working in community settings and with volunteers.

  • Skills in volunteer management, partnering, outreach, networking.

  • Basic understanding of the social determinants of health

  • Familiar with data collection methods.

  • Ability to work well independently and as part of multidisciplinary team.

  • Excellent written and verbal communication skills in English and/or French.

  • Intermediate to advanced computer skills (MS Word, Outlook, Excel and internet).

  • Ability to work flexible hours, including evenings and weekends


Desirable

  • The ability to communicate in a third language is a definite asset.

  • Familiarity with EMRs (Electronic Medical Records)


Language Designation

  • English written and spoken.

  • French – an asset
