CCHC is multi-service Community Health Centre that offers a full range of community services delivered by a diverse team of professionals based on Anti-Oppressive, Collaborative leadership, Equity, Empowerment and Continuous Learning values Our team includes doctors, nurses, social workers, counsellors, dietitians, community developers, health promoters, and outreach workers.
In 2024, CCHC received new funding from Ontario Health East to provide Preventative Care services. We are currently in the process of recruiting two Prevention Specialist and a Preventative Care Program Administrative Assistant.
The Preventive Care Program aims to improve preventive care for equity deserving and underserved populations and ultimately reduce chronic disease incidence and complications by facilitating access to primary care, preventive programs/services, promoting healthy behaviours and screening and assessing broader impacts of holistic preventive care on wellbeing.
The program focuses on modifiable factors of health (e.g., behaviour change) and acknowledges that systemic and structural barriers (e.g., racism, discrimination, colonial policies) impact participants’ opportunities and abilities to accomplish their preventive goals and considers chronic disease risk factors (e.g., tobacco use, alcohol consumption, physical inactivity, unhealthy eating), overall health and wellbeing (e.g., physical, mental, social needs of an individual), social determinants of health (e.g., income, food security, housing, employment and working conditions, education) as well as structural and systemic barriers (e.g., discrimination, disparities in access) impact individuals’ opportunity for health and wellbeing.
