About the Program

Our Urban Health Program is for individuals who are experiencing homelessness, are living on the street, or are at risk of homelessness AND do not have a family doctor or need help re-establishing a relationship with their family doctor.  

Additional Details

Our team includes doctors, nurse practitioners, nurses, social support workers and harm reduction workers who have expertise serving people who are homeless, have experienced trauma and/or are living with concurrent disorders.  


We offer Walk-in Health Clinics on Tuesday and Friday mornings from 9:00-11:30 a.m.


Clients can get help with:

  • Health issues (mental and/or physical), chronic disease management, preventative care, and palliative care

  • Practical supports such as harm reduction supplies, shower access and ID replacement

  • Navigation to mental health and addiction supports, and other social service supports     

  • On-site access to specialty services: psychiatry, diabetes, lung health, and hepatitis C  


This clinic is a temporary clinic.  Our team will work with you and the client to explore health care options to access long term.   


Eligibility for the Urban Health Walk In Clinic:

  • No access to a family doctor or primary care provider

  • Experiencing homelessness (adults, youth and families), living on the street or in a shelter, or unstably housed (for example this may include individuals in a rooming house)

  • New to Canada + not eligible for OHIP or are on Interim Federal Health (IFH) insurance 

  • Involved in the sex trade

  • Recently incarcerated and living in the CCHC catchment area  


*Clients will be triaged to ensure they meet our eligibility criteria  


If you are referring a client to our program we want to work with you and your client for the best possible care. We ask that you accompany your client to their first visit if possible or call ahead of time to communicate how to best meet their needs.
